8 weeks, concept to launch

Outcome

Nurses reported a 22% increase in patient communication in the first 90 days.
96% onboarding success rate when a nurse walked the patient through it.
Spanish-speaking patients can message their care team in their own language.

Rapid Response

Discharged kidney patients need a 14-day follow-up so they don't land back in hospital. 15% were missing that window. I took the fix from manual pilot to an official Salesforce feature.
